Một chai pennyroyal, một chai xô thơm Dalmatia, một chai hạt ngò tây, một chai hồi. Bốn chai nhỏ, bốn phân tử khác nhau, cùng nằm trong danh sách mà dữ liệu an toàn khuyến cáo tránh khi mang thai. Bài này đọc danh sách đó ở cấp phân tử, theo trang thai kỳ của thư viện, và không thay thế lời khuyên của nhân viên y tế.

Một danh sách ngắn trong tủ

Trong một tủ tinh dầu vài chục chai, số chai bị tài liệu an toàn xếp vào nhóm cần tránh hoàn toàn khi mang thai không nhiều, nhưng chúng có chung một đặc điểm: mỗi chai chứa một hoặc hai phân tử ở hàm lượng rất cao, và những phân tử đó đã có dữ liệu độc tính sinh sản. Trang tinh dầu và thai kỳ của thư viện liệt kê hơn hai mươi loại như vậy.

Danh sách đó gồm những cái tên quen và lạ. Hồi, hoa hồi, thì là ngọt, thì là đắng. Xô thơm Dalmatia, bài hương chemotype pinocamphone, trắc bá, ngải Tansy. Pennyroyal, lộc đề, bạch dương ngọt. Savin, hạt ngò tây, hạt thì là Ấn Độ. Quế vỏ, đinh hương nụ, kinh giới. Thư viện ghi rõ danh sách này không vét cạn.

Điều đáng chú ý là danh sách không được sắp theo họ thực vật hay theo mùi. Nó được sắp theo thành phần hóa học. Hai loại cây cùng họ có thể một loại nằm trong danh sách, một loại không, tùy vào việc tinh dầu của chúng chứa phân tử nào. Đọc danh sách vì vậy phải bắt đầu từ phân tử.

Vì sao lại là phân tử

Phân tử tinh dầu nhỏ, ưa chất béo và không tích điện. Theo trang thai kỳ của thư viện, chính những đặc tính đó khiến hàng rào nhau thai không chặn được phần lớn chúng. Một khi đã vào máu người mẹ, qua da, qua đường hít hay đường uống, đa số thành phần có thể đi qua nhau thai và tiếp xúc với phôi hoặc thai nhi.

Thư viện chia các mối quan ngại thành bốn nhóm cơ chế. Nhóm thứ nhất cản trở phôi làm tổ hoặc gây sảy thai, như sabinyl acetate và apiole. Nhóm thứ hai độc với hệ thần kinh đang phát triển, như thujone, pulegone và pinocamphone. Nhóm thứ ba có hoạt tính giống estrogen, như trans-anethole trong hồi và thì là. Nhóm thứ tư ức chế sự hình thành mạch máu mới, như costunolide trong mộc hương.

Rủi ro không chỉ phụ thuộc vào phân tử. Thư viện nêu ba yếu tố cùng lúc: thành phần cụ thể của tinh dầu, liều và đường đưa vào cơ thể, và giai đoạn thai kỳ. Tam cá nguyệt đầu, từ tuần 1 đến tuần 12, là giai đoạn hình thành cơ quan và được mô tả là nhạy cảm nhất.

Pulegone và pennyroyal

Pennyroyal, thư viện gọi là bạc hà hoa, là một cây họ Hoa môi có mùi bạc hà hăng. Cái tên này thực ra chỉ ba loài khác nhau ở ba vùng: Hedeoma pulegioides ở Bắc Mỹ, Mentha pulegium ở châu Âu, Micromeria fruticosa ở Thổ Nhĩ Kỳ. Cả ba có hóa học giống nhau: tinh dầu giàu pulegone.

Trang pennyroyal của thư viện ghi pulegone chiếm khoảng 61 đến 87 phần trăm tinh dầu. Pulegone và chất chuyển hóa của nó, menthofuran, là chất độc gan nghiêm trọng. Hội đồng châu Âu xếp hai chất này vào nhóm độc gan và đặt mức dung nạp hằng ngày chung 0,1 mg cho mỗi kg thể trọng. Trang thành tố pulegone ghi thêm rằng menthofuran còn độc với gan hơn chính pulegone.

Thư viện xếp pennyroyal vào nhóm chống chỉ định nghiêm ngặt trong thai kỳ và cho con bú, qua mọi đường dùng. Trang tinh dầu ghi nhận nhiều ca tử vong do uống tinh dầu pennyroyal, có ca vì nhầm lẫn, có ca vì dùng với mục đích phá thai. Ghi chú cuối trang mô tả đây là tinh dầu nguy hiểm nhất trong perfumery thương mại, được đưa vào thư viện chủ yếu để cảnh báo.

Bài học ở đây là một cái tên có chữ bạc hà không nói gì về độ an toàn. Bạc hà cay và pennyroyal cùng họ, cùng một nét mùi, nhưng thành phần chính khác nhau hoàn toàn. Dữ liệu an toàn đi theo phân tử, không đi theo tên gọi.

Thujone, apiole, sabinyl acetate

Thujone là một ketone hai vòng, gồm hai đồng phân alpha và beta luôn đi cùng nhau trong tự nhiên. Theo trang thành tố của thư viện, nó đối kháng thụ thể GABA A, làm neuron bị kích thích quá mức, và có thể gây co giật. Nguồn chính gồm tuyết tùng đỏ Tây Mỹ, trắc bá, xô thơm Dalmatia với khoảng 13 đến 48 phần trăm alpha thujone, và nhiều loài ngải. Thư viện ghi chống chỉ định trong thai kỳ.

Apiole của ngò tây là một phenylpropenoid ether có trong tinh dầu hạt ngò tây ở mức khoảng 11 đến 67 phần trăm. Trang thành tố ghi liều thấp nhất đã gây sảy thai ở người là 900 mg mỗi ngày trong tám ngày, và nhấn mạnh tác dụng tích lũy: apiole đào thải chậm, còn phát hiện trong nước tiểu mười hai ngày sau lần uống cuối trong một ca ngộ độc. Mức độ quan ngại được xếp là chống chỉ định tuyệt đối.

Sabinyl acetate là một trong số ít ester độc trong tinh dầu. Trên chuột, tinh dầu savin chứa khoảng 50 phần trăm sabinyl acetate ngăn phôi làm tổ ở liều 45 và 135 mg/kg. Nguồn chính là savin, Juniperus sabina, với 19 đến 53 phần trăm. Thư viện ghi liều an toàn trong thai kỳ chưa được xác định, và savin cần tránh hoàn toàn.

Ba phân tử này khác nhau về cấu trúc và cơ chế, nhưng giống nhau ở một điểm: chúng tập trung ở mức cao trong một số ít tinh dầu. Đó là lý do danh sách tránh dùng ngắn, và cũng là lý do nó phải được đọc chính xác, theo tên Latin và chemotype, chứ không theo tên chung.

Nồng độ, đường dùng, thời gian

Bên cạnh danh sách tránh hoàn toàn, thư viện có một bảng thứ hai: những tinh dầu không bị cấm nhưng có giới hạn nồng độ trên da riêng cho thai kỳ và cho con bú. Phần lớn là tinh dầu giàu citral, một aldehyde có mùi chanh. Sả chanh 0,7 phần trăm, màng tang 0,8 phần trăm, tía tô đất 0,9 phần trăm. Thư viện giải thích citral có thể can thiệp vào quá trình tổng hợp retinoic acid, một phân tử tín hiệu của giai đoạn phôi sớm.

Đường dùng thay đổi mức phơi nhiễm rất nhiều. Theo trang thai kỳ, đường uống cho mức hấp thu toàn thân cao nhất và không được khuyến nghị cho bất kỳ tinh dầu nào nếu không có chỉ định y tế. Đường da hấp thu chậm hơn nhưng vẫn đáng kể, nhất là trên diện tích lớn hoặc da tổn thương. Đường hít cho mức thấp nhất, nhưng không bằng không.

Về thời gian, tài liệu tham chiếu của thư viện mô tả cách tiếp cận thận trọng nhất là không dùng tinh dầu trong tam cá nguyệt đầu. Sau đó, với các tinh dầu không có dữ liệu chống chỉ định, mức pha loãng được dẫn là tối đa 1 phần trăm, thấp hơn nhiều so với mức thường dùng cho người lớn. Thư viện cũng lưu ý không dùng nhiều tinh dầu trong nhóm chống chỉ định cùng lúc, kể cả ở nồng độ rất thấp, vì tác động cộng gộp chưa được nghiên cứu đầy đủ.

Giai đoạn cho con bú có cùng danh sách chống chỉ định. Thư viện ghi lượng thành phần đi qua sữa thường dưới 1 phần trăm liều của mẹ, nhưng trẻ sơ sinh chuyển hóa và đào thải chậm hơn người lớn, nên camphor, thujone, pulegone, methyl salicylate, sabinyl acetate, apiole và safrole vẫn được xếp vào nhóm cần tránh.

Hỏi người có chuyên môn

Một bảng dữ liệu an toàn là tài liệu tham chiếu, không phải một chỉ định. Trang an toàn của thư viện nói điều này ngay ở đầu: nội dung không thay thế tư vấn của bác sĩ hoặc chuyên gia y tế. Với thai kỳ, câu đó có trọng lượng lớn hơn bất kỳ con số nào trong bài.

Mỗi thai kỳ có bối cảnh riêng: tiền sử, thuốc đang dùng, những tình trạng mà người ngoài không biết. Thư viện nhắc rằng một số thành phần tinh dầu tương tác với thuốc qua hệ enzyme của gan. Chỉ bác sĩ sản khoa, nữ hộ sinh hoặc dược sĩ theo dõi thai kỳ mới đặt được những con số chung vào một trường hợp cụ thể.

Vì vậy cách đọc hữu ích nhất của danh sách này là dùng nó để đặt câu hỏi đúng, không phải để tự trả lời. Mang theo chai, đọc tên Latin và chemotype trên nhãn, hỏi người có chuyên môn. A short list in the cabinet

In a cabinet of a few dozen oils, the bottles that safety literature places among those to avoid entirely in pregnancy are not many, but they share one trait: each holds one or two molecules at very high levels, and those molecules carry reproductive toxicity data. The library's essential oils in pregnancy page lists more than twenty such oils.

The list mixes familiar and unfamiliar names. Aniseed, star anise, sweet fennel, bitter fennel. Dalmatian sage, pinocamphone chemotype hyssop, thuja, tansy. Pennyroyal, wintergreen, sweet birch. Savin, parsley seed, Indian dill seed. Cinnamon bark, clove bud, oregano. The library notes that the list is not exhaustive.

What stands out is that the list is not ordered by plant family or by scent. It is ordered by chemistry. Two plants from the same family may fall on different sides, depending on which molecules their oils contain. Reading the list therefore has to start with molecules.

Why it comes down to molecules

Essential oil molecules are small, fat loving and uncharged. According to the library's pregnancy page, those very properties mean the placental barrier does not stop most of them. Once in the mother's bloodstream, through skin, inhalation or mouth, most constituents can cross the placenta and reach the embryo or fetus.

The library groups the concerns into four mechanisms. The first interferes with implantation or causes miscarriage, as with sabinyl acetate and apiole. The second is toxic to the developing nervous system, as with thujone, pulegone and pinocamphone. The third is estrogen like activity, as with trans-anethole in aniseed and fennel. The fourth inhibits the formation of new blood vessels, as with costunolide in costus.

Risk does not depend on the molecule alone. The library names three factors together: the specific composition of the oil, the dose and route into the body, and the stage of pregnancy. The first trimester, weeks 1 to 12, is when organs form and is described as the most sensitive window.

Pulegone and pennyroyal

Pennyroyal, called bạc hà hoa in the library, is a mint family plant with a sharp minty smell. The name actually covers three species in three regions: Hedeoma pulegioides in North America, Mentha pulegium in Europe, Micromeria fruticosa in Turkey. All three share one chemistry: an oil rich in pulegone.

The library's pennyroyal page gives pulegone at about 61 to 87 percent of the oil. Pulegone and its metabolite menthofuran are serious liver toxins. The Council of Europe classes both as hepatotoxic and sets a group tolerable daily intake of 0.1 mg per kg of body weight. The pulegone constituent page adds that menthofuran is even more toxic to the liver than pulegone itself.

The library places pennyroyal under strict contraindication in pregnancy and breastfeeding, by every route. The oil page records multiple deaths from swallowing pennyroyal oil, some by accident, some from its use as an abortifacient. A note at the bottom of the page describes it as the most dangerous oil in commercial perfumery, listed in the library mainly as a warning.

The lesson is that a name containing the word mint says nothing about safety. Peppermint and pennyroyal belong to the same family and share a facet of scent, yet their main constituents are entirely different. Safety data follows the molecule, not the name.

Thujone, apiole, sabinyl acetate

Thujone is a bicyclic ketone, with alpha and beta isomers that always occur together in nature. According to the library's constituent page, it antagonises the GABA A receptor, overexciting neurons, and can cause convulsions. Main sources include western red cedar, thuja, Dalmatian sage at about 13 to 48 percent alpha thujone, and several wormwoods. The library records it as contraindicated in pregnancy.

Parsley apiole is a phenylpropenoid ether found in parsley seed oil at about 11 to 67 percent. The constituent page gives the lowest dose known to have caused miscarriage in humans as 900 mg a day for eight days, and stresses a cumulative effect: apiole is cleared slowly, still detectable in urine twelve days after the last dose in one poisoning case. The concern is rated absolutely contraindicated.

Sabinyl acetate is one of the few toxic esters in essential oils. In mice, savin oil with about 50 percent sabinyl acetate prevented implantation at 45 and 135 mg/kg. The main source is savin, Juniperus sabina, at 19 to 53 percent. The library notes that a safe dose in pregnancy has not been established, and that savin should be avoided entirely.

These three molecules differ in structure and mechanism but share one feature: they are concentrated at high levels in a small number of oils. That is why the avoid list is short, and also why it has to be read precisely, by Latin name and chemotype, not by common name.

Concentration, route, timing

Besides the avoid list, the library keeps a second table: oils that are not prohibited but carry dermal limits specific to pregnancy and breastfeeding. Most are rich in citral, a lemon scented aldehyde. Lemongrass 0.7 percent, may chang 0.8 percent, melissa 0.9 percent. The library explains that citral can interfere with the synthesis of retinoic acid, a signalling molecule of early embryonic development.

Route changes exposure a great deal. According to the pregnancy page, oral use gives the highest systemic absorption and is not recommended for any oil without medical direction. Skin absorption is slower but still meaningful, especially over a large area or on damaged skin. Inhalation gives the lowest load, but not zero.

On timing, the library's reference material describes the most cautious approach as no essential oils in the first trimester. After that, for oils without contraindication data, the dilution cited is a maximum of 1 percent, far below levels commonly used for adults. The library also notes that oils from the prohibited group should not be combined, even at very low concentrations, because cumulative effects have not been studied in depth.

Breastfeeding carries the same prohibited list. The library notes that the amount passing into milk is usually below 1 percent of the mother's dose, but newborns metabolise and clear substances more slowly than adults, so camphor, thujone, pulegone, methyl salicylate, sabinyl acetate, apiole and safrole remain on the list to avoid.

Ask someone qualified

A table of safety data is reference material, not a prescription. The library's safety page says so at the outset: its content does not replace the advice of a doctor or health professional. In pregnancy that sentence weighs more than any figure in this piece.

Every pregnancy has its own context: history, current medication, conditions no outsider knows about. The library notes that some essential oil constituents interact with drugs through liver enzymes. Only the obstetrician, midwife or pharmacist following the pregnancy can place general figures in a specific case.

The most useful way to read this list, then, is as a way to ask the right question, not to answer it alone. Bring the bottle, read the Latin name and chemotype on the label, ask someone qualified. When in doubt, the library's conclusion is short: default to not using it. The piece on diluting oils for skin explains how the percentages are calculated.

The list is short because only a few molecules truly matter.

It has to be read precisely because common names mislead.
